PtEverywhere is committed to giving you control over your personal information. Use the form on this page to request deletion of your PtEverywhere account, the personal information associated with it, or both.

We will acknowledge your request within 10 business days and complete verified requests within 30 days. We will verify your identity before acting on your request, and may ask for additional information for that purpose.

If You Are a Patient of a Physical Therapy Clinic

Your treatment records are owned and controlled by your clinic. PtEverywhere hosts these records on the clinic’s behalf as its service provider and HIPAA Business Associate, and federal and state law require clinics to retain treatment records for a minimum period. We cannot delete treatment records without your clinic’s authorization.

Please contact your clinic directly for requests involving your treatment records. If you submit the form on this page, we will forward your request to your clinic and support them in responding to it.

If You Are a Clinic Staff Member or Account Holder

Submitting this form will delete your user profile, login credentials, saved preferences, and marketing contact information from our active systems. Deleting your user profile does not cancel your clinic’s subscription. To cancel a subscription, contact support@pteverywhere.com.

What We Delete

When we delete your account, we remove your user profile and account details, including your name, email address, and phone number; your login credentials; your saved preferences and settings; and your marketing contact information and communication history.

What We May Retain

We may retain limited information where the law requires it. This includes records required for tax and accounting purposes, records needed to honor an email or text message opt-out, and information we must preserve in connection with a legal claim, audit, or investigation. It also includes health and treatment records controlled by a clinic, which are subject to that clinic’s legal retention obligations. Any information we retain is limited to what is necessary for that purpose and is deleted when the requirement ends.

Information removed from our active systems may persist briefly in encrypted, access-restricted backups before being overwritten in the ordinary course. Full detail is available in our Privacy Statement.
