ONLINE AND MOBILE PRIVACY STATEMENT
Welcome to PtEverywhere’s website and/or mobile application (“App”). We respect your right to privacy and value the trust you have placed in us. We work hard to protect the information you provide online.
What Information Does PtEverywhere Collect?
When you register or log into your online PtEverywhere accounts via the App, you may be required to share certain personal information with us, for example, your name and address, or other information that can be used to identify you. We may supplement the information that you submit to us online with other personal information that we legally obtain about you from public and non-public records. When you return to your PtEverywhere accounts, this personal information may be used to “authenticate” or confirm your identity in order to secure and properly restrict access to your online accounts.
Through our websites, we collect information using tiny text files called “cookies”. Cookies track the pages you visit and they make web pages load faster and improve your online experience.
When using certain functions via the App, your location information may be used if your mobile device uses global positioning system (“GPS”) technology, trackers or other location tools. If you allow your location information to be obtained, we will use this information solely to return your estimated location and to provide you with more personalized results and/or services. PtEverywhere does not monitor your GPS tracking or location information and does not save this information.
The App uses images in two different ways. Images that are part of your home exercise program remain on your device for your own use and are not uploaded to PtEverywhere servers. Separately, you may choose to attach an image to a document or to a message with your provider. When you choose to do so, that image is uploaded to PtEverywhere servers and becomes part of the record or conversation you attached it to. We do not access the photo library on your device beyond the images you specifically select.
How We Use Your Personal Information
We do not sell your personal information unless you authorize us, in writing, to do so. Any personal information you provide on our websites or App will be used for the purpose stated on the page where it is collected and in accordance with applicable laws and regulations. To support our business operations and to provide services to you, we may share the information we collect about you with our Customer Service Department, employees and/or third parties that work for us. We may use contact details (including phone number) to send service-related communications such as email or SMS reminders, and to process opt-in/opt-out requests.
Data Retention
We retain personal information only for as long as necessary to provide our services, to comply with our legal, regulatory, and contractual obligations, to resolve disputes, and to enforce our agreements. How long we keep information depends on what it is and why we hold it.
Account information is retained for as long as your account is active. When your account is closed, or when you ask us to delete it, we remove it as described in the section below.
Health and treatment records are hosted by PtEverywhere on behalf of the physical therapy clinics that use our software. The clinic, not PtEverywhere, controls this information and determines how long it is retained, consistent with the federal and state laws that apply to that clinic’s records. We retain it for as long as the clinic instructs us to. When a clinic’s agreement with us ends, we return or delete the health information we hold on its behalf in accordance with our Business Associate Agreement with that clinic.
Payment and transaction records are retained for the periods required by applicable tax, accounting, and audit obligations. Payment card details are held by our payment processors, not by PtEverywhere.
Records of your communication preferences are retained for as long as applicable communications law requires. If you ask us to stop sending you email or text messages, we keep a record of that request so that we can continue to honor it.
Usage information, including cookies, analytics data, and application logs, is retained for a limited period and is then deleted or aggregated into a form that no longer identifies you.
Location information is not retained. It is used on your device only to determine your time zone.
Information removed from our active systems may persist briefly in encrypted, access-restricted backups before being overwritten in the ordinary course. Our internal retention schedules govern the specific periods for each category above.
How to Request Deletion of Your Data
You may request that we delete the personal information we hold about you. To submit a request, visit https://www.pteverywhere.com/data-deletion-request and complete the form, or email privacy@pteverywhere.com with the subject line “Data Deletion Request.”
Please include your full name, the email address and/or mobile number associated with your account, and the name of your physical therapy clinic if applicable. We will verify your identity before acting on your request, and may ask for additional information for that purpose. We will acknowledge your request within 10 business days and complete verified requests within 30 days. If we need additional time, we will tell you why and when to expect completion.
You may also delete your PtEverywhere account by submitting the form above. Deleting your account removes your profile, login credentials, saved preferences, and marketing contact information from our active systems.
When we delete your account, we remove your profile and account data as described above. We may retain limited information where the law requires it, for example records required for tax and accounting purposes, records needed to honor an email or text message opt-out, and information we must preserve in connection with a legal claim, audit, or investigation. Any information we retain is limited to what is necessary for that purpose and is deleted when the requirement ends.
If you are a patient of a PtEverywhere clinic, your treatment records are owned and controlled by your physical therapy clinic. PtEverywhere hosts them on the clinic’s behalf as its service provider and HIPAA Business Associate. We cannot delete these records without the clinic’s authorization, and federal and state law require clinics to retain treatment records for a minimum period. Please direct requests to delete, amend, or obtain a copy of your treatment records to your clinic. If you contact us instead, we will forward your request to your clinic and support them in responding to it.

Does PtEverywhere allow unaffiliated third parties to access data collected on PtEverywhere’s websites?
You may choose to share information about your visits to PtEverywhere’s App or public website, PtEverywhere.com, through email and social networking sites that are not affiliated with PtEverywhere. If you access these sites through PtEverywhere.com, PtEverywhere will allow third-party companies to collect certain anonymous information about your visits in order to provide advertisements about the goods or services they offer that may be of interest to you. These companies use a cookie or third party web beacon to collect this information. No individually identifiable information (such as your name or address) will be made available to these companies.
How We Keep Your Information Safe
PtEverywhere uses many safety measures to make sure your information is secure. Examples include firewalls and data encryption. We regularly review the security of our electronic systems, including our web pages. We check to make sure our security systems stay up to date and work.
Contacting Us
When you email us, please do not include information you want to keep private. This includes information that could be used to identify you. While the email is on its way to us, someone else may intercept or read it. If you see an email address or a link on a PtEverywhere website that opens an addressed email, do not use it to send private information.
We do not require you to communicate with us electronically. Feel free to call us or mail us a letter if you prefer. You can also contact us to ask that a customer service representative call you. Our main customer service phone number is listed on https://www.pteverywhere.com/about-us

Text Messages (SMS)
If you provide your mobile phone number and opt in, PtEverywhere (or our service providers) may send you marketing text messages, including product updates, promotional offers, and other marketing communications. Message frequency varies. Message and data rates may apply.
Consent & Opt-Out
You can opt out of SMS messages at any time by replying STOP to any message. For help, reply HELP. After you opt out, you may receive a final confirmation text and then no further messages (unless you later re-opt in).

Service Providers & Sharing
We may use third-party providers to deliver text messages. Text messaging originator opt-in data and consent will not be shared with any third parties, excluding aggregators and providers of the text message services. We do not sell your mobile number or share it with unaffiliated third parties for their marketing purposes.

To provide you with valuable information, now and then our websites and App have links to third party websites we do not own or control. A link does not mean we approve or endorse a site. We are not responsible for the quality, accuracy, security or privacy practices of sites we do not own. Be sure to read the privacy statement of each site you visit.
Does This Privacy Statement Apply if I’m Not in the U.S. When I Use or Visit PtEverywhere’s Websites or App?
When you use our websites or App from outside the United States, including when you are in the European Union (“EU”), it means you consent to have the personally identifiable information you give us processed outside of the country and, if applicable, outside of the EU.
